Where you'll train
Participants will have the privilege to train at the Mo Ostin Basketball Center — the official practice home of UCLA Men's Basketball.

Facility note: Mo Ostin Basketball Center is UCLA Athletics property. Access and specific areas used may vary based on facility guidelines and availability.

Event Logistics

Waiver & Release of Liability / Assumption of Risk

I understand that participation in basketball training, strength training, conditioning, competition, and related activities involves inherent risks, including but not limited to: falls, collisions, overexertion, dehydration, heat-related illness, sprains/strains, fractures, concussion, and other serious injury or illness. I voluntarily choose to participate (or allow my child to participate) and assume all risks, known and unknown, to the fullest extent permitted by law.

Release

In consideration of being allowed to participate, I (on behalf of myself and/or my minor child) hereby release and hold harmless Dailey Training International / DaileyTraining.org, The Eric Dailey Jr. Experience, event staff, coaches, volunteers, contractors, facility owners/operators (including UCLA-related facilities where applicable), and all affiliated partners from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to participation, including travel to/from the event, except where prohibited by law.

Medical consent

I certify that the participant is physically able to participate. In the event of an emergency, I authorize event staff to obtain medical treatment for the participant. I understand I am responsible for all medical costs and that the organizers do not provide medical insurance coverage for participants.

Code of conduct

I understand participants are expected to follow coaching instructions, respect staff and other athletes, and maintain safe behavior. The organizers reserve the right to remove any participant for unsafe or disruptive behavior.

Media release (photo/video)

I grant permission for photos and videos taken during the event to be used for marketing, promotional, and educational purposes (website, social media, email, ads, and partner promotion) without compensation.

Parent/guardian acknowledgment (minors)

If the participant is under 18, I confirm I am the parent/legal guardian and I agree to this waiver on behalf of the participant and myself.

Free event / limited capacity acknowledgment

I understand this is a free event and that capacity is limited. I understand registration will automatically close once capacity is reached, and that my spot is confirmed only when I receive confirmation from the EDJ Camp team.

Facility notice

I understand facility access and designated viewing areas for parents/guardians may be limited due to facility rules and staff direction.

E-signature consent
